Our team model gives each caregiver a network of support from their coworkers as well as a partner with whom to swap scheduled days or times in the event of illness or other unforeseen circumstances.
Each team has a Team Leader who serves as a liaison between the team, the client, and West Pointe Healthcare management. Our RN offers supervision and coaching for advanced care services. Our caregivers establish a “web of caring” among themselves and with our clients.
We provide graded levels of care to match the changing requirements of our customers, ranging from basic assistance and companionship to more extensive, RN-supervised care, whether it be in your home, in your retirement community, or in a facility.

We maintain a personal care journal for clients utilizing Level II or III care. This journal serves as an ongoing record of the client’s care, offering insights into daily routines and activities. Family members can stay informed, and alternate caregivers can easily follow the client’s personalized care plan. Our caregivers ensure continuity and tailored support.
This journal not only captures routine care activities but also provides a detailed account of daily observations, including patterns of activities, behavior, mood, nutrition, and hydration. It ensures continuity of care and offers an easy-to-follow example of the client’s daily routine for alternate caregivers
